· 课件简介:Conceptual framework for the metabolic syndrome There has been a marked disagreement about definitions of the metabolic syndrome. To some degree, this has been driven by different conceptual frameworks for the metabolic syndrome. In the literature at least 3 underlying concepts about the metabolic syndrome exist. These different concepts are not necessarily mutually exclusive. The first concept is that environmental causes are responsible for the epidemic of the metabolic syndrome. This is the view of the National Cholesterol Education Program (NCEP). If this view is correct, the principal treatment would be behavioral, that is to reduce obesity and increase physical activity. The second view is that insulin resistance is the underlying cause of the metabolic syndrome. This definition is interested in etiology and thus insulin resistance is a required component of the metabolic syndrome as defined by the World Health Organization (WHO) criteria (unlike the NCEP which suggests only any 3 out of 5 criteria are adequate). The WHO criteria would suggest that one could treat obesity and increase physical activity but is also more friendly to the use of insulin sensitizers in non-diabetic subjects. A third concept is that inflammation might be the underlying cause of the metabolic syndrome. If this concept is correct, a very broad range of treatment opportunities exists since statins, ACE inhibitors and ARBs have also been shown to decrease levels of CRP, a marker of subclinical inflammation.
